The advantage of this protocol is that 5 – 10 patients can be operated on in the clinic at the same time on the same day by the same doctor. So even if they charge less, their revenue is high. Still, it is quite risky for your hair grafts.
  2. Type 2 clinics – Doctor makes slits ( channels or holes in the bald area – 15% work of your hair transplant surgery). Assistants do the rest of the work. Slit-making by a doctor ensures that hair will grow in the desired direction because the doctor creates slits or holes. The success percentage in these cases may be 70 – 80 %, but there will always be chances of failure or more donor area damage. The problem with this protocol is that a staff harvest your hair grafts instead of a doctor. Before-after result photos may be fine, but these procedures can utilize 1.5 – 2 times the donor area to produce the same result experts can have using less donor area. Suppose you need a hair transplant again after 5 – 10 years. Then that is challenging in these cases. If a hair transplant fails badly, you cannot regain your donor area. If you go to the expert doctor for a repair, he will again use the donor area. So you may use 2.5 times the donor area to produce the same result in hair transplant failure. The donor area is limited and does not grow back. So even if the first clinic returns your money, you are also at a severe loss in case of significant hair transplant failure. 
  3. Type 3 clinics – The doctor makes slits and harvests grafts from your donor area.
  4. Type 4 clinics – The doctor makes slits in the bald area, harvest grafts from the donor area, and insert grafts there.

Which type of protocols are internationally approved to do hair transplant ?

Only type 3 and type 4 protocols are approved internationally. Type 4 protocol is ideal but takes almost 1.5 – 2 times to perform your surgery, so expense increases significantly. Most clinics do not provide type 4 protocol.

How to choose a clinic for hair transplant in India or your own country

When you choose a clinic, ascertain these factors.

a. What is the role of a doctor in your surgery? Type 3 should be the minimum protocol (the doctor should do harvesting of the graft along with slitting in the bald area ). Refuse type 1 and type 2 protocols ( where the doctor is obeserver or makes only slits in the bald area) 

b. Who will do your surgery? Sometimes famous plastic surgeons or hair surgeon advertise their clinic and results. When you contact their clinic, they tell you that the surgery cost is very high. Degree are obtained after years of training . MBBS degree requires 5.5 years training. MD or MS degree requires 3 years further training after 5.5 years of MBBS. And M.Ch plastic surgery degree is 3 years training after a doctor has completed 8.5 years training already ( 5.5 years of MBBS and 3 years of MS ) . So to obtain a M.Ch. in plastic surgery a doctor needs minimum 11.5 years of training. Whereas a doctor with only MBBS degree can obtain a hair society membership even without 1 day of training. These are hair societies who provide membership based on interest of doctor . To attain these memberships, there are no training. Suppose a doctor is perform only 5 hair transplant in 1 year but he is interested in the field of hair transplant , so he can apply for membership of these societies after paying membership fees of society. So now you are aware of concept that we need around 50 % density of normal hair to look normal in long hair . If you want shorter hair styles you may need more density.

Your normal density is around 80 – 120 grafts per sq. cm .  In hairline area you can go for density of 50  – 60 grafts per sq. cm but if area is more than 30 sq. cm then it better to go with density of 35 – 40 grafts per sq cm after creation of hairline . Some plastic surgeons are biased with FUT Technique and they promote it extensively as better method than FUE.

Is Graft damage rate in FUE more than FUT ?

Yes Damage rate in FUE is more than FUT. In inexpert hands damage in FUE can reach 30 – 50 % also. But in expert hands it varies from 2 – 5 % . The in experienced surgeons damage 30 – 50 % extra donor area to harvest same number of grafts. SO FUE requires really a expertise and You should do FUE only with selected highly trained doctors.

Never go to a clinic where graft extraction is done by assistant or technician.  But in really expert hands the volume of hair is much more in same number of FUE grafts even if you get 2 – 3 % damage. Reason is thicker grafts in FUE as we can select graft before harvesting it . In FUT we cannot select graft so their average thickness is less than FUE grafts.

Is FUE is not from safe donor area ?

Safe donor area is defined a around 7 cm height area where hair are relatively stable until your old age. FUT utilizes just 1.5 cm height. FUE utilizes 5 – 7 cm height. So a Standard FUE procedure of 3000 grafts is also from safe donor area. Request your surgeon to remain confined to safe donor zone.

Is FUT result better than FUE ?

Suppose we are doing 3000 grafts by FUE and 3000 grafts by FUT . Then FUE result will be having better thickness and better volume than FUT. The reason is that when we do FUE we tend to choose better grafts in term of hair thickness and number of hair per graft. In FUT we cannot control the thickness of graft and average number of hair per graft.

In Microscopic hair transplant ( MAGP ) the specificity of FUE is further enhanced due to Microscope use. And graft will have more thickness and more average number of hair per graft. Because of these reason FUE or MAGP will yield better result in same number of grafts . If we want to obtain same result with FUT , we will have to do 20 % more grafts than FUE to gain same volume.

Can trichophytic closure prevent scar formation in FUT technique ?

Answer is No. Trichophytic closure is done so that one or two row of hair grow inside the FUT scar So that it can be hidden if it is not broad. If the FUT scar is broad then trichophytic closure will also not help much.

Is it possible to do FUT without scar ?

You cannot prevent scar formation in FUT but its width will be minimum if strip is taken after assessment of donor area skin laxity and width of strip is less than 1.5 cm . If you have been shown a FUT donor area after 6 -7 months of Hair transplant without scar , then insist to see that patient photo after 2 – 3 years .

Does the life of FUT graft is more than FUE graft ?

No . FUE grafts taken from safe donor area will have same life. If FUE graft is taken from unsafe donor area then its life will be lesser than FUT graft.

  1. 5000 grafts by FUT + FUE combination technique – In this method around 3000 grafts are done by FUT then 2000 grafts are done by FUE to make 5000 grafts available from your scalp . Average 5000 grafts cost in India varies from Rs 125000 – Rs 250000 ( US dollar – 1800 – 3500 )  . 5 % clinics may charge you more than that .
  2. 5000 grafts by combination of scalp grafts and beard grafts – In this method 3000 – 4000 grafts are done by FUE from scalp . Rest grafts are done by Beard as donor area. The result will not be as good as FUT + FUE combination. Because FUE grafts from beard are single so 1000 grafts from beard provide you volume equivalent to around 600 grafts from scalp.
